COMPANY AUDITION REGISTRATION
Dancers who want to audition for a Company are
required to have a completed copy of the following
forms before they will be allowed to audition:
- Company Audition Form (signed)
- Company Requirements Form (read)
- Liability Release Form (signed)
- Estimated Prices Guideline (read)

Once you have auditioned and been accepted into a
Company, please complete the following forms:
- Registration Form (sign)
- Company Requirements Form (sign)
- Company Required Classes (read)
- Liability Release Form (from audition)
- Estimated Price Guideline (by request only)
Please Note: New Company Contracts are updated
and must be signed and returned to Melissa Larson or
Kim Machado.
Company, please complete the following forms:
